PCGS 1971-D Washington quarter indented by a Roosevelt dime planchet. The coin has an indent that is the size and shape of a dime planchet that was struck into the quarter and "spread out" during the strike.
The error occurred when a dime planchet was fed into a Washington quarter press, and was fed between the dies along with a Washington quarter planchet, and they were struck into each other.
