ANACS 1971-D Washington quarter 2-coin mated pair set, with one coin being a struck thru fragment, and the other coin struck thru a fragment of broken-away clad layer, which broke away from another coin and was struck into this coin. Both the coin and the fragment escaped the Mint together.

 

This is a scarce error type, as almost always with this type of strike thru, the fragment is lost to the struck thru coin.
