NGC 1996 proof track torch reverse Olympic die, used for striking silver dollar track and field commemorative coins for the 1995/1996 Atlanta Olympic games. The die was used for striking some of those coins, and then was retired and "cancelled" with an "X" shaped across the obverse side.
