PCGS 1997-P Jefferson nickel with a Lincoln 1c planchet indented into the coin's reverse side.

 

The indent was caused by a Lincoln cent planchet which was fed into the press and struck while resting underneath this coin. The result is an indent from the cent planchet. 

 

A rare error type, and tough to find.
