PCGS (1965-1998) Washington quarter struck on a nickel blank, and also 35% off-center. An off-center off-metal, and with the blank weighing 4.93 grams.

The coin is struck off-center on a Jefferson nickel blank due to a nickel blank being fed into a Washington quarter press and struck off-center. Scarce to have the two error types together.

MS62 with lustrous surfaces. Some small spots on the coin.
